Design and implementation of the 'SAFER' near-miss management system for development of a databank on basic accident factors

Objectif

The main aim of this project is to set up a databank for 'near-miss' data, complete with the associated recording and analysis modules. The system as a whole (called SAFER) is to be used as a management tool for safety staff at Hoogovens Ijmuiden to begin with. If successful it can serve as a model for similar systems in other ECSC industries.

Analyses of accidents and breakdowns in plants have revealed how large a role people play not only in bringing about such undesired incidents, but also in preventing possible serious consequences developing from errors or breakdowns. Both human error and human recovery therefore determine to a considerable extent not only whether a dangerous situation arises but also whether such a situation develops into an accident or other adverse situation.

Consequently, an important contribution to structural improvement of a plant's safety level can be made by pinpointing not only factors conducive to or even provoking human error, but also factors which make for timely and correct human recovery. By both eliminating or reducing 'error factors' and introducing new 'recovery factors' into the production system (and reinforcing those already existing), the problem of plant accidents is thus tackled from two angles.

By recording and analysing not only actual accidents (which are quite rare) but also the numerous 'near-misses', it is possible to build up a statistically reliable databank, within a relatively short period, containing both error and recovery factors. Statistical analysis of the data thus collected will then provide management with not just a qualitative but also a quantitative picture of the basic (non-) safety factors which have the greatest influence on the occurrence or prevention of accidents at work.
