Effects of group auditing on improving drug therapy in primary care; a randomized controlled trial

The Slovakian participant will test an auditing programme for improving drug therapy, developed and tested in the original project for primary health care, in a specialised outpatient setting, a system for health care used widely in Central European countries. The auditing programme is using analysis of clinical judgment in conjunction with an assessment of knowledge and prescribing practices according to standardized methods. It will be applied to asthma management in outpatient clinics, in contrast to the original project not to cystitis, because in specialised outpatient clinics this is not feasible and because of time constraints. The objectives are to improve the effectiveness and efficiency of the treatment of asthma, to harmonise auditing programmes and to involve Slovakia in the development of new methods for health care research. A protocol has been developed for a randomized controlled intervention study. Asthma is a relevant health problem, expert opinion about its treatment has changed considerably (NIH international consensus), but actual practice lags behind, even more so in Slovakia then in EU countries. The Slovakian research team combines good research experience with an organisation enabling the implementation of the programme effectively. The auditing progranme is directed at small groups of pneumologists working at outpatient clinics. The groups receive feedback information about their own knowledge and prescribing practices, measured at the baseline registration and compared to the international consensus, followed by information about the way the physicians weigh and use different patient characteristics when deciding on a treatment, based on the results of clinical judgment analysis. The effect is tested at the level of knowledge and actual prescribing behaviour. Knowledge is measured in structured standardized questionnaires in conformity with the original project. Prescribing data are collected from health insurance data, using standardized methods. Randomization is done at group level. Outcome variables are compared pre- and postintervention in the experimental and control group.
