This review covers the principal physics issues associated with the production and confinement of high energy alpha particles in an ignited tokamak plasma. The three main sections deal with possible loss processes for alpha particles, the influence of alphas on MHD modes, and thermonuclear instabilities driven by the alphas. A brief summary of outstanding problems is provided, together with some suggestions for future research.
