Major Accident Reporting System : Lessons learned from accidents notified
According to the requirements of the Seveso Directive, the national authorities of the Member States must notify the CEC of major accidents involving dangerous substances. These accident notifications are stored in the MARS (Major Accident Reporting System) database of the Institute for Systems Engineering and Informatics at Ispra. They are analysed in order to classify the accidents according to various parameters and identify the lessons to be learned from them. This report presents the lessons learned from notified accidents by grouping them into five categories: - design/construction-related lessons; - operation/maintenance-related lessons; - lessons learned from handling emergencies; - lessons learned from mobilisation after an emergency, and - specific lessons related to a particular substance or process.
