Abstract

Root cause analysis (RCA) methodologies have been conceived for the retrospective analysis of the accidental sequence to analyse system failures and human actions involved, in order to determine which ones involved errors and for what reasons. RCA approaches are conceived for both system failures and human errors but this paper concentrates on the basic needs for a tool for assessing the causes of human errors. An RCA framework is outlined and event sequence analysis and causal analysis are discussed.
