A methodological framework for root cause analysis of human errors
The methodology proposed for the root cause analysis (RCA) of human errors enables the incorrect behaviour of operators to be detected and the possible causes to be identified. It is based on a paradigm of human behaviour and a taxonomy for human errors. The use of the paradigm is important to detect the causes of cognitive errors, whereas the use of a taxonomy is essential to distinguish between internal and external causes affecting human performances. The theoretical formalisation of the methodology for the retrospective analysis of incidents/accidents is presented.
