Failure of a fuel irradiation capsule with inactivated cooling and instrumentation due to its unnoticed location during start-up
This paper investigates a failure that occurred in one of the fuel irradiation experiments located in the pool side facility of the HFR whilst the reactor was being brought into full power. The experiment was incorrectly located in the high flux zone of the pool side facility, with its instrumentation and cooling systems inoperative. This situation passed unnoticed during the reactor check-out before start-up, leading to overheating of the irradiation capsule and subsequent fuel melting and activity release to the pool water.
