Abstract

Root cause analysis (RCA) methodologies have been developed for the retrospective study of accidents or incidents, with the specific goal of analysing human error contributions to causation. The methodology proposed aims to identify the initial operator error and then to find the triggering cause. Two case studies are presented: the A320 accident near Strasbourg airport in January 1992 and the DC-9 accident near Zurich airport in November 1990.
