Common cause failures and human factors
The main objective of the Human Reliability Assessment (HRA) is the identification and quantification of the most relevant human-machine interactions affecting the development of an incidental sequence. The quantification is made in terms of probabilities and frequencies of occurrence of unsuccessfully performed tasks/missions, due to inappropriate or inadequate human behaviour. In the Probabilistic Safety Assessment (PSA) of a plant, the probabilities of a mission failure are usually combined with the probabilities of a system failure in order to achieve the overall reliability evaluation of the man-machine system for well-defined initiating events and accident progression scenarios.
