The aim of this thesis is to finalise a methodological approach to the analysis of the causes of human error. Using a cognitive model and a classification of errors based on this model, it is possible to reconstruct the cognitive processes of an operator leading through a causal chain to an unsafe act, which links typical errors of human cognition, such as bad planning, distraction, etc, to underlying errors.

This reconstruction is carried out through video analysis of human activity. This prototype has been incorporated in an integrated multimedia environment, the architecture of which has been designed at the same time as the tool. This environment depends on a high level multimedia system for digital set-up, and on a database for the collection of analytical results. A study was required to define how the system could be used to serve as a basis for the environment; a prototype has been created in Visual Basic and Access in a Windows 3.11 environment. .
