Abstract

This paper focuses on the type and structure of data and databases that must be applied to support such methods for performing probabilistic safety assessment studies and root cause analysis, in consideration of the new supervisory and management roles assigned to operators.

In particular, a new classification system and database in the domain of aviation is presented. The associated taxonomy is analysed with respect to its ability to represent an accident following the most modem theories of accident causation that relate front line operators with the whole organisational structure.
