Objective
ASSESSMENT OF DIFFERENT METHODS DESIGNED TO ESTIMATE THE EFFECTS OF HUMAN FACTORS IN RISK AND RELIABILITY ANALYSES. THESE METHODS WILL INVOLVE BOTH QUALITATIVE AND QUANTITATIVE METHODOLOGIES. THE ANALYSIS WILL BE UNDERTAKEN UNDER THE FOLLOWING HEADINGS, APPLICABLE TO BOTH FUNCTIONAL AND EMERGENCY PROCEDURES:
1) READ PROCEDURES.
2) PRODUCE COMMENTARY ON APPARENT PROCEDURE ERRORS, OPERATOR ERRORS AND PROBLEMS.
3) PROVIDE LIST OF QUESTIONS TO BE PUT TO THE OPERATORS.
4) INDICATE THE REQUIREMENTS FOR ADDITIONAL ACCESS TO OPERATORS AND PLANT DATA.
5) TASK ANALYSIS.
6) IDENTIFICATION OF ERROR MODES AND ERROR MODELLING.
7) RESTATEMENT OF HUMAN ERRORS AND PROBLEM AFTER RESPONSE BY KWU TO ITEMS 3 AND 4 ABOVE.
8) ADVICE TO BE GIVEN ON ANY PROCEDURAL CHANGES.
9) ASSESSMENT OF
(I) SYSTEM UNAVAILABILITY;
(II)PROCEDURE ERROR PROBABILITY;
(III)OPERATOR ERROR PROBABILITY.
THE NATIONAL CENTER OF SYSTEMS RELIABILITY (NCSR) CONTRIBUTION TO THE HUMAN FACTORS RELIABILITY BENCHMARK EXERCISE WILL UNDERTAKE TO ANALYSE FUNCTIONAL TEST PROCEDURES PROVIDED BY KWU AND RELATING TO THE EMERGENCY FEED SYSTEM.
THE ANALYSIS OF THE PROCEDURES WILL EMPLOY BOTH QUALITATIVE AND QUANTITATIVE HUMAN FACTORS METHODOLOGIES.
THE ANALYSIS OF OPERATOR PERFORMANCE WILL BE UNDERTAKEN USING ONE OR MORE METHODS FOR THE ASSESSMENT OF HUMAN RELIABILITY.
