Objective

Deliverables

  • Outcome measures and importance weights

    A list of outcomes and indicators to measure outcomes of ICC models specifically targeting patients with multi-morbidity plus five sets of weights that indicate the importance of these outcomes from the perspective of five stakeholder groups: patients, informal caregivers, professionals, payers and policy makers.

  • Detailed description of 16 best practice ICC models

    An analysis of the thick descriptions of the 16 best practice ICC models with a special focus on how the process of care delivery is designed around the patient and how the implementation is supported by ICT applications, self-management, and new professional roles in best practices.

  • Conceptual framework

    Conceptual framework for describing and evaluating ICC models for multi-morbid patients plus selection of promising ICC models for further study in WP2

  • MCDA Framework

    A methodological framework to perform a comprehensive evaluation of ICC models for patients with multi-morbidity using MCDA and a description of study designs that ensure the outcomes can be causally attributed to the ICC model under investigation, given different types of data availability.
