Integrated solutions for heart failure patients
In Europe, 15 million people live with heart failure and many of them with multimorbidity. The care model in place for these patients implies one specialist for each comorbidity. The medication becomes complex to manage and risky if not handled properly. Severe episodes imply costly hospitalisation and decline in quality of life, penalising both the patient and the health care system. The EU-funded TIQUE project will make extensive use of cutting-edge technologies to reduce avoidable hospitalisations and preserve the health status of the patient. Smart devices at home, artificial intelligence to anticipate and prevent crisis, and implementation of new care pathways are main components of the value-based service to be designed.
