Post abortion family planning services in China: a demonstration - intervention project to increase contraceptive use and to reduce unwanted pregnancies and induced abortions

The purpose of the research project is to define optimal ways to introduce postabortion family planning services (PAFP) in urban areas in China in order to reduce the rate of unwanted pregnancies and induced abortions. About 10 million induced abortions are performed annually in China; 52% as a result of contraceptive failure and 48% from non-use of contraception. Thirty percent of all abortion are repeat abortions. This high prevalence seriously affects reproductive health in China, particularly in ad olescents and youth. The introduction of PAFP is known to be an effective strategy to reduce the number of repeat abortions. Operations research is needed to design interventions adapted to the Chinese socio-cultural context and to evaluate the effecti veness of different models. In a first phase, qualitative research methods will be used to develop quality PAFP and a model for its introduction in China, and to better understand the reasons for abortion, barriers to family planning and attitudes of pro viders and clients towards postabortion contraceptive use. In a second phase, the acceptability and the effectiveness of these services will be explored. In this perspective, an intervention study will be carried out in three Chinese provinces. Two dif ferent types of PAFP will be implemented in randomly selected abortion clinics: a comprehensive package, including counselling and provision of contraceptives and an essential package limited to information and referral to existing family planning servic es. The effectiveness and reasons for failure or success will be measured by comparing qualitative and quantitative indicators pre- and post intervention and between both intervention groups. The study will result in scientific information, publications and policy recommendations on introduction of cost-effective PAFP in China, which will be of interest to Chinese health authorities as well as to the global reproductive community.
