Mortality of British steel workers over an extended follow-up period

Objectif

Analyses conducted to date, with a follow-up of 43 months, have shown that useful conclusions can be drawn from existing data on the mortality of steelworkers. The aim of this project is to answer the following questions :

a) Does longer follow-up support the view that blast furnace and sinter operatives in 1971/73 were at particular risk of subsequent mortality from lung cancer ?
b) If so, to what extent is this difference specific to particular plants, regions or age-groups of workers ?
c) To what extent, if at all, are blast furnace and sinter operatives also at greater risk from other causes of death ?
d) What evidence, if any, does a longer follow-up provide regarding other differences in mortality possibly related to occupation within the steel industry ?

The study will be based on 16 100 men whose mortality was analysed in relation to occupation in 1990. These men were "current" workers at the time of data collection in 1971/73. Their occupation at that time was recorded by means of the BSE line number and their vital status up to 31 July 1977 was established successfully.

The available information permitting identification on computer files will be transmitted, in a mutually acceptable format, to the Department of Social Security which will confirm that the workers in question is alive or will supply the date of death.

All the data will be protected in accordance with the legal requirements on data protection, and access will be restricted to personnel directly involved in the project. Comparisons will be between groups of occupations and the identification of individuals will not be possible.

METHODS AND MEANS BY WHICH THE AIMS ARE TO BE ACHIEVED

The men will be identified on computer files by surname, full forenames (or initials), date of birth, national insurance number, plant, occupation in 1971/73 and, for those who died prior to 31 July 1977, date and (primary, underlying) cause of death, coded according to the eighth revision of the ICD (International Classification of Diseases) system.

The statistical analyses will include standard person-years-at-risk comparisons of the cause-specific mortality of the cohort as a whole with that of the general male population in Britain, taking account of age, region and calendar year. There will also be comparison of mortality between subgroups of the cohort, defined by combinations of occupation, age and region or works, using Poisson regression methods.
