The HTLY European research network (HERN) antenatal seroprevalence & perinatal transmission study

Objective

To determine the seroprevelence of HTLV-I infection in pregnant women and their children within the European Community using standardised methods between centres.
To examine the mother-to-child transmission rate of HTLV-I in Europe and to validate the testing of eluates from Guthrie cards (dried blood spots).
To determine HTLV-I seroprevalence.

The Human T-cell Leukaemia/Lymphoma Virus (HTLV) - type I is the only known infectious cause of leukaemia in humans. HTLV-I is endemic at an unknown level in Europe. Transmission occurs horizontally through sexual intercourse and blood transmission and vertically from mother to child principally through breastfeeding. In hyper-endemic countries the prevalence of HTLV-I is predominantly maintained through vertical transmission. In Europe little is known about vertical transmission. In data accumulated by HERN the prevalence of HTLV-I in women attending ante-natal clinics is 0 - 20/1000 in unselected populations.

The objectives of this proposal are:
(1) Screening. To establish these roprevalence of HTLV-I infection in women attending ante-natal clinics in Europe;
(2) Methodological. To ensure comparable findings by standardisation of reagents;
(3) Nested transmission study. To determine the likely number of infected children, who have an increased risk of developing Adult T-cell Leukaemia? National HTLV reference laboratories from Belgium, France, Germany, Italy, Portugal, Spain, Sweden and the United Kingdom have identified clinical collaborators able to provide the sera from women attending antenatal clinics and linked anonymised Guthrie card samples from live births. To ensure the reliability of the survey it is proposed to study 250,000 sera during 24months. The results of this survey will be published within 12 months of the completion of the laboratory tasks. An algorithm for the serodiagnosis of HTLV-I infection suitable for European seroepidemiology studies has been defined by a group of virologists, serologist and epidemiologists in 1994.
