EN Doppler linear Processing for Hydraulics Imagery New System
1 OBJECTIVES
Cerebrovascular disease is a major source of stroke and death consecutive to the reduction of cerebral blood flow by artery stenosis or thrombosis, embolization of plaques and clots, intracranial haemorrage and hypertension. Most of these sources of stroke can be treated by medical or surgical means when detected early. For instance, it is of major importance in terms of public health and cost benefit to be able to evaluate by non invasive approach the potential risk of atheromateous plaques in the arteries supplying the brain as they are at the origin of a large number of strokes. Ultrasonic techniques (Bmode, Doppler) are more and more widely used for the early diagnosis and for the follow-up of cerebral vascular disease originating in the arteries of the neck (carotid and vertebral arteries), and detection of micro emboli in intracranial vessels by transcranial Doppler. Several very experienced tearns have shown that it was possible to refer patients to carotid surgery without X Rays angiography, only on the basis of ultrasonic examination. It becomes therefore necessary to evaluate very precisely the potential of ultrasonic techniques for the diagnosis, prognosis and follow-up of carolid disease, and to develop retrievable diagnostic procedures in order to reduce as much as possible the sonographer dependance of the ultrasonic examination. Three major objectives have been identified for the Dolphins project:
1. to determine the most relevant haemodynamic and flow velocity parameters characterizing the degree of carotid stenosis, based on Doppler flow velocity measurements (continuous wave Doppler, pulsed wave Doppler, color coded Doppler including amplitude and frequency detection, Doppler enhancement with contrast agents). Data will be collected at different arterial sites: upstream and downstream to the stenotic area, as well as the sile of the stenosis itseif.
2. to characterize the atheromateous plaque using several paramelers measured on Bmode and color coded Doppler images. Among these parameters we can list:
- dimension of the plaque including volume
- texture of reflective tissues (from homogeneous to heterogeneous)
- echogenicity (from hypo or anechoic to echodense or hyperechoic)
- movements of the plaque induced by changes in flow velocity, arterial pressure and relative translator, of the vessel during each cardiac cycle.
3. to incorporate the results of the studies (signal processing, image processing, modified hardware) in existing machines in order to improve the accuracy and the reproducibility of the examinations, and to reduce the role of human factors involved in the ultrasonic study of carotid artery disease. The objective will be of major importance for increasing the role of ultrasonic techniques in the management of cerebrovascular patients.
The objectives of the proposal mesh well with those of Biomed2 workprogramme and its priority research tasks. They are primarily concerned with the needs of the community and consumers, and with the transfer of fundamental research into clinical practice and industrial development. The objectives focus on important issues including cardiovascular diseases, public health, rising health care costs, the need for high quality health care and the improvement of health technologies. They involve interaction between the disciplines of basic research in physics and engineering, industry and clinical applications
