To propose a methodological framework for the evaluation of epidemiological and economical aspects of organized screening programme for breast and cervical cancers.

The systematic screening of breast and cervix cancer is a public health measure which has increasingly aroused the interest of both the individual European member States and the Community as a whole. The numerous regional programmes undertaken in the different countries will undoubtedly result in the acquisition of the experience which is indispensable for a more ambitious implementation of wide spread programmes. But systematic screening programmes cannot be implemented without a 3-dimensional evaluation: medical, epidemiological and economic.
An analysis was made of the epidemiological and economical aspects of an organized screening programme for breast and cervical cancers. Data concerning each of the 12 Member States were presented. This study has no intention of dictating a specific structure to which Member States should adhere. Each Member State will develop its own particular screening programme in keeping with its epidemiological situation, its sociocultural modes and medical and health services. When the European Committee of Oncologists proposes a specific model, then it will be employed for calculation purposes. For the other cases, the model is based on hypotheses.
Methods
1. Appraisal of the demographic and epidemiological context by recovering all the available data (census, incidence, mortality).
2. Estimation of the expected advantage in term of years of life saved.
3. Analysis of all the items able to influence this estimation: participation rate, false results (negative or positive) interval cancer rates, poor quality control, untargeted population, etc.
4. Cost-analysis.
Estimation of the number of screening tests and biopsies needed, calculation of the needs in staff and equipment; cost of organization, information and training, quality control.
