Objective
The most sensitive method for early non-invasive detection of fatal developmental anomalies is ultrasound examination between weeks 11-14, when embryonic development has been completed. Besides standard optometry, examinations include measurement of uncial translucency to screen for chromosomal anomalies, cardiac defects and also for determination of chorionicity of multiple pregnancies. Despite our extensive knowledge it has not yet been determined whether Tran abdominal or transvaginal ultrasound is the better tool for examining fatal anatomy between weeks 12 and 14, and whether 3D and 4D ultrasound can improve the visualisation of fatal organs at this gestational age. 2000 randomly selected patients attending routine first trimester ultrasound screening will be examined. Standard measurements will be carried out using 20 Tran abdominal ultrasonography as gold standard. In the same session pregnancies will also be examined by 2D transvaginal, 40 Tran abdominal and 4D transvaginal methods. It will be attempted to assess the role of Tran abdominal versus trasvaginal imaging in 3D and 4D ultrasonography in first trimester ultrasound, and will work out a detailed diagnostic protocol for ultrasound examinations at 11-14 weeks of gestation. The role of 3D and 4D ultrasound will be evaluated for the early first trimester detection of cardiac and morphological anomalies. The applicant will have the opportunity to gain experience in 2D, 3D and 4D first trimester prenatal ultrasonography and diagnostic workup of screened cases carrying fatal malformations. He will carry out a substantial number of the examinations under close supervision by an expert in fatal medicine. He will also gather experience in setting up and carrying out a research project, in data collection and analysis, and scientific presentation of work results.
