Roadmap on navigating the complexities of enabling innovative technologies in transitional care

Includes the results from survey questionnaires and interviews on enablers and barriers of innovations in transitional care (M4). The roadmap will be updated based on UCs execution findings by M24 in D1.4.

Report that includes all measures and activities for disseminating and communicating the project and reaching out to target audiences.

Project Management Handbook

Project management handbook based on the Horizon Europe regulations to be used internally by the consortium determining organisational aspects and reporting. The Handbook will include quality assessment, risk mitigation and data management plan.
