Objective
Diagnostics are the biggest gap in the cascade of care for priority infectious diseases in sub-Saharan Africa (SSA). Lack of accessible and affordable diagnostics contributes to poor health outcomes, drives antimicrobial resistance, and impedes timely responses to emerging epidemics. The transformative potential of point-of-care molecular host response diagnostics has been recognised but until now, none have been developed specifically for the needs of SSA. We have developed a lab-on-chip platform to detect malaria, bacterial, viral, and TB infections (and co-infections) based on differences in their host gene-expression response. In this project we will take this platform beyond proof-of-concept, evaluating it in relevant clinical settings to determine the cause of febrile illness in 1000 children in Burkina Faso and Ghana. Through engagement of stakeholders across the translational pathway, we will also assess and optimise usability, impact on care pathways and public health, regulatory approval processes, local manufacturing, and sustainable funding and access models. In parallel we will build capacity in diagnostics research and innovation and train a cohort of future leaders in the field. This PATHFINDER project will provide evidence for a disruptive approach, with potential to transform the diagnostic ecosystem, and have wide-reaching impact on healthcare delivery in SSA.
