Objective
Background
Population ageing is advancing rapidly in Europe, with Central and Eastern European (CEE) countries experiencing faster transitions due to socio-economic change, fertility decline, and migration. Oral health problems are the most common chronic conditions in older adults, with care home residents disproportionately affected by oral diseases, impairing nutrition and quality of life, but evidence on care home oral health promotion strategies is lacking. Therefore, robust scalable public health interventions which can be applied across EU level are required.
Objectives
This study will aim to test the feasibility of an oral health promotion programme for older care home residents in Vilnius, Lithuania, to inform a future trial. Objectives are: (i) co-designing the intervention with stakeholders; (ii) implementing a randomized controlled trial; and (iii) evaluating programme's feasibility, recruitment, retention, and acceptability.
Methods
The intervention, co-developed with residents, staff, dental and geriatric professionals, and municipal partners, will include staff training, use of the Oral Health Assessment Tool, and daily assisted tooth-brushing with fluoride toothpaste. A cluster randomized controlled feasibility trial will recruit 120 residents from 12 care homes randomized to intervention or control groups (1:1), with a six-month follow-up. Outcomes will include clinical indicators (number of teeth, caries, plaque, bleeding), assessment of oral health needs (OHAT tool), and oral health-related quality of life. Semi-structured interviews will explore programme's feasibility and sustainability.
Expected Impacts
This study will generate evidence on scalable oral health promotion that can inform guidelines and future research to improve oral health and quality of life among institutionalized older adults across Europe, reducing future burden on health care systems and associated dental treatment costs.
