Objective
ExoCordOA is a flagship initiative to advance secretome-based therapies through two complementary pillars: (1) the clinical development of Exo-101 for osteoarthritis (OA), and (2) the resolution of systemic bottlenecks that limit broader adoption of secretome medicines. Exo-101, a first-in-class extracellular vesicle (EV)-enriched secretome derived from umbilical cord blood, has demonstrated robust anti-inflammatory, immunomodulatory, and regenerative effects in preclinical models of OA. ExoCordOA will advance Exo-101 GMP-manufacturing, GLP preclinical safety validation, and all the regulatory and clinical activities needed to finalise Phase I and II clinical trials during the project lifetime. Ultimately, this project will advance Exo-101 towards Phase III post-project, potentially positioning it as Europe’s first clinically validated secretome therapy for OA. In parallel, the project addresses the systemic bottlenecks that hinder broad adoption of EV-based medicines. It will establish a GMP-compliant manufacturing process, develop integrated quality control combining AI-enhanced optical fingerprinting with multi-omics profiling, and support implementation of clinically relevant potency assays based on deep EV characterization in biological fluids. Furthermore, by convening a regulatory forum of international experts, ExoCordOA will provide an evidence-based framework to inform future approvals of secretome therapies. Bringing together biotech innovators, clinical trial experts, hospitals, CROs, GMP manufacturers, regulators, academics, a cord blood bank, market experts and patient representatives, the consortium has the expertise and infrastructure to deliver a disease-modifying treatment for OA while building the foundations for Europe’s leadership in secretome-based therapeutics.
