Objective
Thoracic Endovascular Aortic Repair (TEVAR) is a minimally invasive treatment for thoracic aortic pathologies. Despite its widespread clinical adoption, preoperative planning is manual and experience-based, relying on measurements from computed tomography (CT) images. This simplified approach does not account for complex interactions between device and vessel. As a result, device selection is uncertain, contributing to complication and reintervention rates up to 30%, with significant clinical and economic consequences. TEVAR-Twin technology addresses this unmet need by transforming TEVAR planning from static geometric evaluation into a predictive and simulation-driven process. TEVAR-Twin will build a digital twin platform that reconstructs a 3D model of the patient’s aorta from CT imaging and virtually deploys commercially available stent-grafts using finite element simulations. The platform will introduce evidence-based decision support into routine TEVAR planning without disrupting existing clinical workflows enabling clinicians to compare multiple intervention strategies, providing both visualization of post-deployment configurations and clinically relevant metrics. The technology has been preliminarily validated on real patient cases, demonstrating high agreement (>90%) with post-operative outcomes. This project will advance the platform through structured technical verification, validation to generate QMS and CE-certification evidence. The final outcome will be a commercially ready and clinically validated medical device integrated into existing planning practice.
By creating the first clinically validated digital twin platform dedicated to TEVAR, AllStent positions itself in a new market segment in MedTech, allowing scalable impact across Europe and expansion to other endovascular applications. The technology has the potential to reduce complications, reinterventions and associated costs, while enhancing clinician confidence and patient safety.
