ICT for ageing well
Improving quality of life for older people and support independent living though integration of health and social care
12 partners are cooperating in the CommonWell project, aiming to deliver ICT-enabled health and social care services in four Member States of the European Union. The integrated services are to support the effective management of chronic disease, and to address issues which affect independence, such as reduced agility, vision or hearing, in order to significantly improve the quality of life for older people and their families.
A total of 400 users across four locations in Europe will receive the newly integrated services for at least twelve months. The results of evaluating the pilot operation will be used to extend service provision and promote the wider uptake of this model of care across Europe.
Milton Keynes and Eindhoven are the pilot locations that will initially address chronic disease management for older people suffering from Chronic Obstructive Pulmonary Disease (in Milton Keynes), and Chronic Heart Failure (in Eindhoven). The CommonWell services will improve communication between health and social care providers, helping to reduce anxiety and improve health outcomes for people with chronic conditions and those who need support when leaving hospital.
FASS and Johanneswerk are the sites focussing on improving services for independent living for older people. FASS currently operates the largest centre for social alarm in Europe with over 90,000 clients across Andalusia in Spain. Johanneswerk runs several hospitals, a social alarm service and provides domiciliary care. CommonWell promises to support integrated care delivery in both organisations.
The project will integrate and evaluate ICT-enabled healthcare and social care services capable of significantly improving the quality of life of older people and their families. The CommonWell service integrates on common open platforms services previously provided by multiple organisations on separate proprietary platforms. Owners and providers of the service will be Local Authorities, public, charity and not-for-profit social care and healthcare service providers in EU regions.The service infrastructure will be implemented and maintained by Europe's leading provider of home service platforms, Tunstall, supported by new entrant into European eHealth provision, HomMed. While meeting the needs of older people and public policy objectives, these organisations have the capability to build and sustain service infrastructure across Europe. Conformance to standards under development by Continua will enable multiple providers especially SMEs to participate in an expanding market.Costs will be met by enabling greater numbers of older people to be served using the same time and resources, by increases in service quality and improved outcomes and by savings through multiple use of common platforms. The primary components will be web services provided in conformance with a Software Oriented Architecture, Continua-conformant home platforms, vital signs, capture devices, smart home detectors / actuators, delivered through internet and high reliability PSTN. Services will be piloted with 400 users in Milton Keynes (UK), Bielefeld (DE), Granada (ES) and Eindhoven (NL)
