Final Report Summary - TAHITI (Improving therapy and intervention through imaging)

TAHITI project scientific objective has been to design, develop, and validate new imaging technologies to improve the performance and safety of therapeutic and interventional procedures. Improving therapy and surgery planning has been approached using multimodal imaging strategies, lesion and dysfunctional tissue characterization, efficient segmentation and image fusion techniques. Image guided therapy and intervention improvements have been pursued by designing the integration of new imaging modalities in the surgical workflow, providing new techniques to compensate for undesired motions as well as innovative methods and instruments to integrate new intraoperative imaging devices. Follow-up and therapy outcome is a major concern has been also tackled using innovative methodologies to quantitatively assess and predict recovery and prognosis after therapy or surgery. Different concrete scenarios have been considered within the frameworks of interventional cardiac therapy and surgery and soft tissue organs therapy. More concretely some of the main achievements are:
Cardiac Therapy and interventions
- New image processing pipelines and tools have been proposed in order to be used in cardiac therapy surgery planning, as well as some tools to allow image guided procedures.
o Proposal and development of a new method for post infarct scar segmentation and analysis of Phase Sensitive Inversion Recovery Images in the Assessment of Myocardial Infarction. Presented at ISMRM 2012-
o Proposal and development of a new multimodal MR imaging protocol along with analysis tools to determine area at risk. Presented at ISMRM 2012 and published in MRM journal 2014
o Proposal of a new method and tool to analyze DE-MR images to provide intuitive information to electrophysiology procedures by means of a derived surface, Journal papers published in Heart Rhythm and JACC: Clinical Electrophysiol.
o Proposal and development of a new method and tool independent of fiducial markers to fuse MR derived information into angiography procedures- Presented at IEEE CinC 2013, Jounal paper published Atienza et al Circ.-Arrhythmia Electrophysiol. 2013.
o Proposal and development of a new method to efficiently segment the RV and LV in 4D MR data.
Presented at IEEE CinC 2012 and Journal paper in Med. Image Anal. 2015.
o Study and proposal of new catheter marker devices for interventional MR, presented at SCMR and ISMRM conferences 2016 and Journal paper in Med Biol Eng Comput 2014.
Soft Tissue Interventions
- New image processing pipelines and tools cystoscopy have been proposed and initially tested, in the fields of liver therapy and surgery as well as in endoscopy procedures.
o Proposal and development of a panoramic view for endoscopic procedures such as cystoscopy.

- Proposal and development of a new method to non-rigidly register MR Primovist data with CT datasets for liver surgery planning. These methods allows identifying metastasis in the surgical planning framework- Journal paper. Medical Image Analysis 2014
- Proposal and development of a new method to segment healthy parenchyma in liver CT image for appropriate surgical planning and volumetry- Presented at EMBC 2012
- Proposal and development of a deformable registration of the liver for image guided abdominal procedures based on liver vessel information.
- In vivo imaging effects of new drugs for Chemotherapy Induced Peripheral Neuropathy, Journal Paper published in the Journal of Peripheral Nervous System 2016.
- Study of new potential applications of XFM tool as well as other multimodal combinations of 3D pre-operative data with real-time during the procedure in soft-tissues and abdominal interventions.
- Proposal of a electrom beam IORT dosimetry tool and a pipeline for image guided IORT using a 2D to 3D registration framework. Presented at CARS 2015 and published in Phys Med Biol 2014.

We have made our contributions aiming at the following impacts of the current project:
a) The development of new techniques and methodologies to assist in the new therapy and surgical procedures will clearly improve the position of EU partners towards technology transfer to the healthcare technology industry.
b) To improve the healthcare of European citizens by providing new and safer means of therapy and surgery.
c) To increase the skills and knowledge of the EU partners in order to better perform clinical translational research implying new technologies.
d) This project is a multidisciplinary initiative allowing the exchange of scientists among four research institutes with international recognition in medical imaging and biomedical engineering impacting in education and research of excellence.

TAHITI-IRSES-2010-269300 integrates the work of four institutions from Europe, Universidad
Politécnica de Madrid, Fraunhofer, Bogacizi University, Universidad Carlos III de Madrid and from USA, Johns Hopkins University and funded by European commission (FP7- Marie Curie Actions- IRSES) . The enclosed figure illustrates part of the achievements of TAHITI project.
