Final project report. This document will provide the relevant project information regarding the whole the project, following the template provided by the EC.

Based on the quarterly monitoring activities, an analysis of the service, ‘Final report on the optimisation of services and processes’ document will contain lessons learnt and good practices to be shared among the partners via our internal repository and update any templates, questionnaires or document as needed.

Intermediary report on Communication and Dissemination Activities (se abrirá en una nueva ventana)

The ‘Intermediary report on Communication and Dissemination Activities’ will contain a review of all communication based initiatives and actions undertaken throughout the first stage of the project M1-M18. Lessons learnt will be identified and if needed, communication plans for the second stage M18-M36 will be actualized.

End users’ need analysis and Service programme document (se abrirá en una nueva ventana)

This document will identify and characterize in detail the beneficiaries of the eHealth Hub support, together with other relevant stakeholders and networks. It will be done in close collaboration with the development of the Dissemination and Communication Plan. The results of the focus group and survey will be analysed and become the basis of the ´End users’ needs analysis and Service programme document´ which details the deployment of the 7 eHealth Hub services, including approach, delivery team and planification for each service. It will also state the eligibility and selection criteria for each service, and the planned follow-up for traction actions for each project initiative including Recruitment Activities. The Advisory Board members will be confirmed at this stage.

The Business Plan will be based on the validated business models of both the support structure and the portfolio of eHealth Hub services - those deployed during the project lifetime but also those identified during its execution. The Business Plan will include a market analysis, a development and implementation plan, a marketing plan, business projections and a financial plan. It will leverage and be in close alignment with the final version of the Dissemination and Communication plan.

‘Final report on eHealth Hub service deployment’ will contain a summary of actions undertaken during the project, focusing on the actions carried in the second stage of the project M18-M36, as well as the lessons learnt throughout the experience and the development of the project.

Exploitation Plan (se abrirá en una nueva ventana)

The eHealth Hub Exploitation plan aims to identify and structure the activities that the partners will put in place to maximize the chances of sustainability after the end of the EU funding. It will be based on the project exploitation strategy defined by the partners after interacting with the market, starting at the focus group and survey at the beginning of the project, and continuing during the deployment of services and recruitment activities. The plan will: - Identify the main target customers and beneficiaries from the key stakeholder types. - Produce an exhaustive list of the key exploitable resources (assets) produced during the project lifetime, and how to economically maximize their potential. - Compile the foreseen exploitation activities. - Incorporate activities to increase scalability.

Corporate identity communication material (se abrirá en una nueva ventana)

The ´Corporate identity communication material’ will serve to improve the brand awareness and share information about the project’s mission among the targets. A logo, flyer, posters, and roll-ups will be designed according to this visual corporate identity as well as presentations and deliverable template to be used in external and internal communication activities.

Quality management plan (se abrirá en una nueva ventana)

The ‘Quality management plan’ contains the strategic management strategies which consists of relating the consortium activity orientations beyond the end of the project with the short-term actions as managed technically by the Coordinator and each of the Work Package leaders. Strategic management tasks include: - Periodic validation of the consortium targets. - Communication of these orientations to all the project stakeholders using the dissemination actions in particular with regards to the plans for the exploitation of project results during and after the project conclusion. - Adjustments of the development trajectory in case of unforeseen events detected by a continuous surveillance of the project environment (technical, technological, economic, social…).

Internal processes deployment plan (se abrirá en una nueva ventana)

The processes and supporting monitoring tools will be setup by INNO at the project start with contribution/feedback from all partners to ensure good coordination, communication and exchange of lessons learnt between partners. These will include an intranet with knowledge management tools, the organisation of semi-plenary and plenary meetings, regular teleconferences, internal best practice repository, short periodic internal reports, online questionnaire for services’ evaluation, exchanges and involvement of the Advisory Board, templates, dissemination procedures and protocols, etc. All internal processes and tools will be detailed in the ´Internal processes deployment plan´.

Project Communication and Dissemination Plan (se abrirá en una nueva ventana)

The ‘Project Dissemination and Communication Plan’ is the strategic document to promote eHealth hub activities and services. It will include, among others: the main communication and dissemination goals, a brief description of the contents to disseminate (which information should be disseminated), the target groups (who should be informed) and who would support the dissemination (partnerships), the communication tools/instruments to be used for different audiences (how to spread knowledge), as well as the time plan (when) and the strategy to overcome any barriers that could negatively affect the project’s communication. The dissemination plan will also detail the approach of the specific Recruitment activities and Multiplicator reports.

The ‘Final report on Communication and Dissemination Activities and Post Project Communication and Dissemination Plan’ will contain a review of all communication based initiatives and actions undertaken throughout the. Lessons learnt will be identified and post project communication activities will be defined focusing on dissemination strategic actions to be undertaken after the projects conclusion, in order to have a follow-up and knowledge spread of projects results.
