Objetivo
The aim of the project is to carry out a proportional mortality study establishing the various causes of deaths among workers in the Basque country's integrated steelmaking plants. In particular, it will :
a) compare the mortality figures obtained with those for the population in general, ascertaining whether or not there is excess mortality for different causes of death, focusing attention on those which are traditionally attributed to the steel industry : cancer of the lung, kidney, bladder, bone marrow, prostate, skin, stomach and nasal cavities, ischaemic heart disease, respiratory insufficiency and industrial accidents ;
b) select and apply indices to estimate "years of life lost" as a result of the various causes of death ;
c) establish patterns of occupational background and risk with a view to identifying mortality risk factors and evaluating the risks which can be attributed to the different factors.
The research will start with the tracing and collection of death certificates received by the compulsory life insurance company. Subsequently, in conjunction with AHV's insurance department, the details of all persons covered by an insurance policy will be examined and the degrees of reliability and representativity of the information obtained from the company will be checked.
The first tentative approach will be followed by a control case study of causes of death which need to be examined in greater depth to establish a cause-and-effect relationship ; the study will be restricted to causes of death which are over-represented in the SPMR (standardized proportionate mortality radio) indicator and are linked with work in the integrated steel industry.
The purpose of the third phase will be to estimate the differences in the incidence of different causes of death, especially deaths from cancer. This would involve a retrospective study of medical records and information on tasks performed. The aim would be to estimate the "Standardised mortality radio"(SMR).
METHODS AND MEANS BY WHICH THE AIMS ARE TO BE ACHIEVED
The estimator will be Mantel-Haenszel's standardised odd radio, adjusted for stratification of the confounders.
The reference data will be those of the Autonomous Basque Community and those of the Spanish population as a whole.
The causes of death shown by the proportional analysis to be significantly excessive and the causes associated in the literature on the subject with work in the steelmaking sector would be simple to study by taking as a comparison group subjects matched by age and sex, selected from their own cohort.
