D1.1 Will deliver two practical tools. These tools will support (a) senior citizens and informal care givers, and (b) care professionals in care transition decisions. The tool are designed to impact on the quality of care.

Care pathway for the management of falls in nursing homes

D1.2 Is a care pathway for the management of falls in nursing homes. The care pathway will be designed with a view to preventing unnecessary transitions for nursing homes to hospitals.
