The operative microscope is the standard visualization means used for all ear surgeries. The main disadvantage of the microscope is the need to work within its 'line of sight' – meaning that although most of the middle ear is visible through the ear canal, many recesses are hidden by the walls of the ear canal. In recent years, endoscopes have become more popular for middle ear surgery since they provide visualization superior to that of the surgical microscopes allowing the surgeon to complete more complex tasks via the external ear canal (transcanal approach), reducing the need for soft tissue incisions and bone drilling. However, drawbacks of endoscopic surgery in the middle ear include extremely challenging one-handed operation technique, reliance on endoscope motion to assess depth perception, inability to observe the tip of angled endoscopes and heat generated by the light source projected in close proximity to sensitive structures such as the facial nerve.
