REDUCTION OF DOSE IN X-RAY DIAGNOSTICS BY THE CHOICE OF THE OPTIMAL SCREEN-FILM COMBINATION

Objectif

THE AIM OF THE PROJECT IS TO ESTABLISH THE THRESHOLD AT WHICH THE SENTIVE SCREEN-FILM SYSTEM CHOSEN BY THE RADIOLOGIST PRODUCES A LOSS OF INFORMATION.

ALTHOUGH IT DEPENDS ON THE SYSTEM USED, AN INCREASE IN SENSITIVITY GENERALLY MEANS A DECREASE IN THE MODULATION TRANSFER FUNCTION (MTF) AND AN INCREASE IN NOISE. CONVERSION OF THESE PARAMETERS (MTF AND NOISE) INTO SPECIFIC RADIOLOGICAL IMAGES BY MEANS OF DIGITAL IMAGE PROCESSING, AND ESTABLISHMENT BY THE RADIOLOGIST OF THE THRESHOLD AT WHICH THERE IS A LOSS OF IMPORTANT IMAGE INFORMATION WILL MAKE IT POSSIBLE TO DETERMINE THE EXTENT TO WHICH THE MFT CAN BE REDUCED AND THE SENSITIVITY INCREASED (REDUCTION OF DOSE) WITHOUT LOSS OF INFORMATION.

THE RESULTS ARE EXPECTED TO DEPEND ON THE OBJECT AND THE IMAGING SYSTEMS USED. THE EXPERIMENTS WILL BE CONFINED TO THREE OBJECTS (THORAX, SKULL, JOINT) AND TO THREE SYSTEMS SO AS TO LIMIT THE EXTENT OF THE STUDIES REQUIRED. IF THE METHOD IS IN PRINCIPLE SUCCESSFUL, IT IS PLANNED TO EXTEND IT TO OTHER OBJECTS AND SYSTEMS AT A LATER DATE.
A study has been carried out into the physical image quality of X-ray screen film systems used in conventional radiology and the visual detectability of diagnostically relevant image details. The image quality parameter used was the Wiener spectrum, which quantitatively describes the image noise properties of screen film systems. The study investigated the influence of 2 different Wiener spectra on the visual detectability of lung modules in chest images. Original chest radiographs were digitized by a scanner, digital images stored and lesions simulated by a digital image processing system. Two sets of images presented to radiologists differed in image quality by image noise. It was shown that a Wiener spectrum which was about 50% larger did not cause a significant reduction in detectability of lung nodules in X-ray chest images. This indicated that an X-ray screen film system with a higher speed could be used without loss of diagnostic accuracy and that demands on the measuring accuracy for the determination of Wiener spectra may be relatively low.
THE PROJECT IS IN FIVE SECTIONS:

1. CREATION OF THE SOFTWARE FOR THE SYNTHETIC PRODUCTION OF NOISE IN THE IMAGE PROCESSING SYSTEM, COMBINATION OF IMAGES AND NOISE AND FILTERING OF IMAGES IN THE FOURIER REGION FOLLOWED BY REVERSE TRANSFORMATION (5 MONTHS).

2.MEASUREMENT OF IMAGE QUALITY PARAMETERS (MTF, NOISE, CHARACTERISTIC CURVE) IN THE THREE SYSTEMS SELECTED AT THE PTB AND IN A MEDICAL PRACTICE IN WHICH THE RADIOLOGICAL IMAGES WILL ALSO BE PRODUCED (2 MONTHS).

3. "READING" OF THE RADIOLOGICAL IMAGES BY MEANS OF A SCANNER AND PRODUCTION OF AUTHENTIC COPIES (1 MONTH).

4. PROCESSING OF IMAGES BY THE IMAGE PROCESSING SYSTEM, PRINTING OF THE MODIFIED PICTURES, EVALUATION BY RADIOLOGISTS (4 MONTHS).

5. EVALUATION AND SUMMARY OF RESULTS (1 MONTH).
