Evaluation of transluminally placed endovascular grafts as treatment for abdominal aortic aneurysms

Objetivo

- To develop optimal technique for endoluminal aneurysmal repair and to evaluate its efficacy and safety.
- To identify HEALING characteristics of stentlgrafts. - To identify optimal implantation site.
- To characterise short term (< 30 days) and long term (> I year) morbidity and mortality.
- To develop improved implantation devices - To develop an European registry for endovascular stent/grafts.

Basic requirements for the procedure will be evaluated in experimental studies in pigs. These studies will be addressing healing (tissue ingrowth, vessel wall reaction and stent/graft incorporation). The value of intravascular ultrasound in determining exact deployment site of the proximal stent will be determined.

The stentgraft systems available will be improved technically by elaboration of a one stage system permitting simplified procedure and deployment of the stent/graft. Stent design will be optimised for firm and durable fixation to the aortic wall. In a co-ordinated effort between the departments involved in the project important clinical aspects of the procedure will be evaluated. Postoperative relationship between aneurysmal diameter and completeness of exclusion (leakage), optimal imaging modalities pre-, intra- and postoperatively, significance of and mechanisms behind a systemic inflammatory reaction after aneurysmal exclusion, procedure related morbidity and mortality and quality of life will be evaluated. An European registry for endovascular aneurysmal procedures will be established. The combined experimental and clinical developments will optimise the safety and efficacy of endovascular stent/graft exclusion of abdominal aortic aneurysms.
