Objectif
Several centers in Eastern Europe collaborate substantially in multicenter phase III clinical trials.
In 1993 Eastern European centers entered 446 patients in EORTC clinical trials.
The vast majority of these patients came from Croatia, The Czech Republic, Slovakia, Hungary, Poland and Slovenia.
The participating Eastern European centers contributed to an important amount to clinical trials regarding treatments for lung cancer, breast cancer, gynaecological cancers, genito-urinary cancer, leukemia and other cancers.
For these common cancers, economic evaluations are currently being carried out or have been planned for the near future with support through the Biomedical and Health Research Program. Key economic data included in the Case Report Forms for a range of ongoing and planned clinical trials (e.g. protocols 55931 in ovarian cancer and 08923 in lung cancer) will be available for most ofthese Eastern European Centers as for Western European centers. Little is known about the relative costs of treating patients with cancer in Eastern European centers neither about differences in treatment patterns and costs between these centers. The same, and even more stringent economic restrictions on the supply of medical services exist in these countries however and the need for sound assessment of costs versus benefits of treatments which are proposed for general adoption after rigorous clinical research, is as urgent as in EC countries.
For Eastern European countries which are in the process of building up modern health care delivery systems, systematic economic evaluations could substitute for blunt cost containment measures and lead to more optimal allocation of health care investments and hence to more health for more patients achieved with the available financial means.
Extrapolation of cost-effectiveness outcomes from a Western context to an Eastern context is not straightforward and no data exist which can indicate how such an extrapolation has to be done. Hence the specific situation in Eastern Europe requires local clinical and economic input when performing economic evaluations. More than for EC countries, investment is needed in collecting basic economic information and gaining know-how concerning costing and financing mechanisms. Central methodological expertise is necessary as well to analyse the results of the local field research and put them into context. At the same time the economic evaluation know-how presently being built up at the EORTC Data Center and in EORTC clinical cooperative groups will be shared with Eastern European centers and there will be spinn offs in both directions regarding mechanisms to ensure efficiency.
