Tele-Medicine Platform to support Home Rehabilitation based on Internet Technologies

Objective


In the envisaged scenario, where a rehabilitation treatment is prescribed, the physician, in co-operation with the physiotherapist, identifies a personalised rehabilitation program for the patient, taking into account the patient status and needs. At the centre, the rehabilitation protocol is accurately defined, based on actual patient status; using the Rehabilitation process management tool the rehabilitation period, the frequency of visits, possible additional examinations, is accurately planned, based on patient’s personal needs.

Based on patient’s condition, the period in which the treatment needs to be performed at the rehabilitation centre can be reduced to the minimum; the remaining treatment is performed at patient’s home, using Rehabilitation machines that are connected to the Internet via a modem. Through the Supervision system and the Internet connection, the Rehabilitation machines receive the session parameters and transmit the patient’s data. The specialist can then monitor the patient’s performance in real time of the ongoing session, and can graphically display the patient’s parameters to identify any possible problem. All data are also stored in a Central repository to allow an accurate review of the results and an adjustment of the rehabilitation protocol if needed. In the subsequent sessions, the Supervision system will download the updated parameters.
