The aged in home care project

Objective

Despite Home Care (HC) is among the fastest growing segments in the health care sector, there is no information about the characteristics of Services, patients and outcomes nor shared knowledge regarding the best models of care. This Project aims to define the characteristics of Home Care patients and Services in 11 European courtiers. Baseline characteristics of both patients and Services and year follow-up data will be collected by trained personnel and centrally scored in a database, the Community Care Data Centre. Data will be shared among partners via Intranet connection. The analysis of the one-year clinical and functional outcomes will enable the possibility to design and propose reference model of Home Care, not available at present.
