Project description
Helping patients connect with doctors
The future of European healthcare is person-centred. Digital advances in communication and information technologies (ICT) are providing patients with easier and broader access to e-health solutions. Relying on telemedicine and mobile health, personalised healthcare not only reduces time spent in hospital but assists in the recovery. The EU-funded HSMonitor project will apply the chronic care model to identify support needs for hypertensive patients at all stages. Its integrated ICT enabled hypertension care solution is based on a design. Suppliers will be evaluated following each procurement phase, from open market consultation and specification of architecture, to the development and testing of the prototype, and a trial phase with 500 patients and 150 professionals.
