Objective
BrainPredict addresses one of the most critical unmet needs in neurocritical care: early and reliable prediction of cerebral vasospasm (CV) and delayed cerebral ischemia (DCI) after aneurysmal subarachnoid haemorrhage (SAH). Across Europe, approximately 58,000 people each year suffer SAH. The downstream consequences of CV/DCI are devastating: 25–50% of survivors remain disabled, and only 30–45% ever return to their previous level of work. Mortality following SAH approaches 50%, with CV and DCI driving a large share of fatal and long-term neurological deterioration. Yet no clinically reliable, early-stage solution exists to identify high-risk patients during the crucial first hours, when proactive intervention could prevent irreversible secondary injury. BrainPredict introduces a fundamentally new approach by fusing computational neurofluidics, volumetric imaging intelligence, and explainable predictive analytics -overcoming long-standing scientific and technological constraints. Unlike conventional models that rely on static radiological grading, BrainPredict links imaging features to mechanistic physiology, capturing how cerebrospinal fluid (CSF) and blood interact to determine the clearance of blood-breakdown products that trigger vasospasm. The solution combines AI-based CT analysis, personalised fluid-dynamics modelling, and an explainable prediction layer - forming a new mechanistic paradigm in cerebrovascular risk modelling, introducing quantitative biomarkers - such as CSF-to-haemorrhage ratios, territorial surface load, and simulated washout efficiency - that more directly reflect the biological processes driving vasospasm and ischemia. By enabling early risk stratification, targeted monitoring, and preventive treatment, BrainPredict has the potential to support timely, personalized care for SAH patients, reduce the incidence and severity of DCI, and deliver a transformative clinical and socioeconomic impact for one of the deadliest forms of stroke.
