Patients die because deteriorations are missed. There is a huge unfulfilled need for better patient monitoring, diagnostics and reaction everywhere in healthcare. A reason is that, despite advancements with remote monitoring devices, the current care model still relies on short “snapshots'' examinations, which leave a lot of dangerous conditions missed, untackled, or tackled too late. At Checkpointcardio, we are enabling the switch from a “Snapshot” model to a “full movie” model where patients and carers are fully and constantly connected.
We have set up the first of its kind 24/7 remote observation medical centre that, thanks to a wearable device and the use of AI, constantly monitors and manages healthcare delivery for more than 50 000 critically ill patients, already saving hundreds of lives. We now want to replicate the model for patients suffering from chronic diseases, multiplying both our business opportunity and the impact we have on people’s lives.

From a technical point of view, the company reached an optimized version of the Archangel system for chronic patients monitoring. We performed a comprehensive analysis of different watches and wristband devices, which led to the construction of SpO2 sensors and to the design of a bracelet. Key results included the integration of a core temperature measurement sensor - tested on different patients and through different environmental conditions - and the design of a new device able to receive data from all devices and to forward these data to the server. Finally, a novel database was established to homogenize information from spot-check medical devices and high-resolution data units. The introduction of optimized protocols has amplified the flexibility of data transmission, enabling remote adjustments from intervals of 10 minutes to real-time live streaming. In this period, Check Point Care also performed an updated FTO analysis and structured a sound and comprehensive plan for disseminating and communicating results to target audiences. For the AI part, tasks included annotating and structuring existing and new patient files, retraining AI-core algorithms to incorporate additional digital biomarkers for chronic patient management, creating new models for detecting physiological state deterioration, and validating these models on test data for deployment in a working prototype environment. The core AI algorithms were enhanced to utilize more parameters from the Digital Twin, increasing the total to 100, and models were validated on both historical data and new patient cases. Clinical protocols established a workflow for the Virtual Ward to manage deteriorations in chronic patients, with the AI system aiding in automatic detection of patient state and therapy effectiveness.
Remote health management solutions have been a topic in research and policies for more than 15 years. However, we are still missing in our homes connected clinical grade devices combined with the ability to get immediate medical services as soon as we need it.
Wearables on the market have fragmented functionality, provide nonmedical or only spot-checks with limited parameters measured and rarely truly connected with medical care professionals.
Our system saves lives by detecting deterioration earlier, as it is able to detect deterioration events hours before the event was noted clinically. As compared to normal practice of the hosting clinical institutes, our system allows 90% decrease in patients deaths, thanks to: i) 24/7 monitoring of patients' condition and real-time analysis and monitoring enabling earlier detection by a dedicated medical team at the Virtual Ward, ii) the establishment of dedicated clinical protocols for managing emergencies, inform the treating clinical team and coordinate their response through our Virtual Ward.
Finally, our solution is by design suited to facilitate incorporation of remote monitoring into actionable clinical decisions. The solution is easy to implement for the treating clinical team. During emergencies they could focus all the effort on providing care, guided by our Virtual Ward suggestions and comprehensive information.
In this project we will pilot the system for at-home management of chronic patients: this will proceed following a two step approach. In the first step: the system will be the object of iterative refinement in all its components in conjunction with the deployment within our own Medical Center. This will allow streamlining of operations to smoothly advance with a complete system. In the second step: the system will be deployed in collaboration with Hospitals and home care providers within our network to independently verify its performance.
