Public health models, application for planning and cost-effectiveness studies public health models

Objectif

A rational public health policy places great emphasis on information gathered from epidemiological research. However when this information has to be used for priority setting in a specific population in a specific European country the epidemiological data need to be transformed into a decision making model to weigh costs and potential health benefits for that particular situation. Public health models have been developed to integrate epidemiological data for such decision making purposes.
Recently the Netherlands, the UK, Denmark and Sweden have been using a computer simulation model called PREVENT, developed in the Netherlands in 1988. It is a cell-based simulation model that can estimate the health benefits for a population of changes in risk factor prevalence due to trends and interventions over a maximum period of 50 years, both in terms of proportional changes in disease specific incidence and in terms of absolute changes in such parameters as disease specific and total mortality.
With this project we propose to create an international forum to provide mutual support for modellers to discuss issues of methodology and data requirements It will also provide a platform for modellers and policy makers using such models to discuss the potential of public health modelling at a European level. Since it will build upon the existing experience of the various partners with the PREVENT projects it is likely to very rapidly yield comparable results. At the same time the combination of a senior and a junior researcher in each team and the possibility of junior researchers to spend some time in one of the other institutes will allow for a new generation of public health modellers to be thoroughly trained in a European context. We strongly believe that the various experiences in this project will enhance the development and the use of public health modelling within Europe

The project proposed here can be divided into three individual projects. The first consists of the development of a common PREVENT version for four different EU countries and using that to generate comparable scenario's to evaluate potential alternative preventive interventions for cardio vascular disease. The second project consists of extending PREVENT to include outcome measures based on morbidity and costs. This is already underway in an explorative stage for the Netherlands using the outcomes of the much more detailed disease models in NIMPH. In this project we will explore the possibility of transferring the NIMPH variables essential to the PREVENT I Plus model for another population, the UK population. The PREVENT Plus model would allow for a policy comparison of improving preventive versus curative programmes for cardio vascular disease. The third project will directly compare the PREVENT model with a micro simulation model POHEM.
