Objective
ALTHOUGH COMMUNITY BASED STRATEGIES FOR THE CONTROL OF MALARIA HAS BEEN IN THE PROCESS OF DEVELOPMENT FOR MANY YEARS, THEY HAVE NOT BEEN YET ADOPTED AT THE LEVEL OF GOVERNMENT HEALTH SERVICES. THESE STRATEGIES INCLUDE: HEALTH EDUCATION (MORE USE OF MOSQUITO NETS, ADEQUATE DRUG TREATMENT, ETC.), COMMUNITY ACTIONS (REMOVAL OF MOSQUITO BREEDING SITES OR THEIR TREATMENT WITH BTI, A HIGHLY SELECTIVE LARVICIDE; IMPREGNATION OF BEDNETS WITH PERMETHRIN) AND INDIVIDUAL USE OF A NEWLY DEVELOPED REPELLENT SOAP. THE EFECTIVENESS OF THESE MEASURES HAS BEEN TESTED UNDER RESEARCH CONDITIONS BY THE PROPOSERS IN ECUADOR AND PERU. IN ORDER TO INTRODUCE THIS PROGRAM AT A NATIONAL LEVEL, THE NEXT STEP IS TO TEST IF, AND UNDER WHICH CONDITIONS AND COSTS, GOVERNMENT HEALTH SERVICES ARE ABLE TO ACHIEVE SIMILAR RESULTS. THEREFORE, LOCAL HEALTH SERVICES IN THREE STUDY AREAS IN ECUADOR, PERU AND COLOMBIA WILL BE INSTRUCTED ON HOW TO ORGANIZE TRAINING WORKSHOPS FOR VILLAGE HEALTH WORKERS AND HEALTH AUXILIARIES. THESE INDIVIDUALS WILL THEN CARRY OUT THE COMMUNITY ACTIONS UNDER THE SUPERVISION OF THE HEALTH STAFF.
THE BASELINE AND OUTCOME MEASUREMENT, BEFORE AND AFTER THE INTERVENTION, WILL BE MADE THROUGH FORMAL HOUSEHOLD INTERVIEW SURVEYS AND INTERVIEWS AMONG HEALTH STAFF, FOR BOTH INTERVENTION AND CONTROL GROUPS. IN ADDITION, THE DETERMINATION OF PARASITE PREVALENCE AND ANTIBODY LEVELS IN THE POPULATION AS WELL AS A SURVEILLANCE SYSTEM FOR MALARIA ATTACKS WILL ENABLE A DESCRIPTION OF THE MALARIA SITUATION BEFORE AND AFTER THE INTERVENTION TO BE MADE FOR BOTH GROUPS. THE MAIN OUTCOME OF THESE MEASURES WILL BE: IMPROVED KNOWLEDGE AND BEHAVIOUR IN THE HEALTH SERVICES AND GENERAL POPULATION; DECREASE OF MOSOUITO BREEDING SITES; AND DECREASE IN THE INCIDENCE OF MALARIA. COSTS ESTIMATIONS WILL COMPLETE THE STUDY.
