Payment mechanisms, efficiency, and quality of care in public and private hospitals in Thailand

Objective



OBJECTIVES: The proposed project aims to investigate the impact of different payment mechanisms on hospital behaviour and how this is affected by hospital ownership. The investigative research will also be used to stimulate health staff to bring about improvements in the hospital management systems, particularly through the development of quality assurance programmes and the strengthening of management information systems. %lmethods: three public, three private non-profit and three private for-profit hospitals, which are comparable in terms of size, casemix and facilities will be selected from the bangkok area. four study components are envisaged: i) macro-level study of hospital efficiency and quality: investigation of efficiency through ratio analysis and hospital bed census, and investigation of quality through the application of quality of care indicators and focus group discussions with patients; ii) micro-level analysis of tracer conditions: four cer conditions will be selected and a detailed examination of efficiency and quality of care in the different payment groups made .to examine efficiency, treatment costs will be directly estimated. quality of care will be examined from two perspectives, that of the professional (using standard of care norms) and that of the patient (using in-depth interviews); iii)development of hospital information systems: after an initial review of the hospital information systems and identification of information needs of managers, improvements no existing systems will be formulated and implemented in those hospitals wishing to collaborate; iv) development of quality assurance mechanisms: the first two study components provide a snapshot of hospital quality care, by involving hospital staff in this work it is hoped to stimulate interest in the development of a hospital based quality assurance system. support will be provided for the development of such a mechanism through technical assistance, training and small development grants to hospitals. %loutputs: (i) which payment mechanisms enhance quality and efficiency, (ii) comparative quality and efficiency data in public and private hospitals,(iii)model hospitals with strenthened management systems.
