Adaptive deep brain stimulation for Parkinson’s disease
Parkinson’s disease presents a significant challenge in managing motor and non-motor symptoms, with existing deep brain stimulation (DBS) systems offering limited flexibility in response to changing conditions. Traditional DBS systems deliver a constant level of electrical stimulation, regardless of the patient’s needs. However, this approach may not always be optimal for patients whose symptoms fluctuate throughout the day. With this in mind, the EIC-funded AlphaDBS project addresses this issue by incorporating adaptive DBS (aDBS) technology. Unlike conventional DBS, AlphaDBS adjusts stimulation levels in real time using local field potentials (LFPs) from neuronal activity. This closed-loop system continuously monitors beta-band activity to provide personalised, dynamic treatment for better symptom management in Parkinson’s patients.
Objective
Differently from other DBS systems on the market, the AlphaDBS can be programmed with two different types of stimulation: conventional DBS, which is implemented also by other DBS systems, and adaptive DBS (aDBS) that automatically adjusts the energy delivered to patient using a biosignal, local field potential (LFP), in a close loop configuration. LFPs represent the synchronous presynaptic and postsynaptic activity of neuronal populations and can be recorded with the macro-electrodes routinely implanted for cDBS. In Parkinson Disease, LFPs (especially in beta-band) are specifically modulated by dopamine intake, movement execution and preparation, and that they encode non-motor functions, such as decision making. The beta band power represents the input variable to the adaptive algorithm, implemented by the AlphaDBS system, which, every second computes a new stimulation value.
