Supporting the Establishment of a Cancer Biobank for Jordan and its Neighbouring Countries Through Knowledge Transfer & Training

Final Report Summary - KHCCBIO (Supporting the Establishment of a Cancer Biobank for Jordan and its Neighbouring Countries Through Knowledge Transfer & Training)

Executive Summary:
King Hussein Cancer (KHCC) is a non-governmental and not-for-profit, specialized cancer centre in the Middle East that treats both adult and pediatric cancer patients from Jordan and the neighboring countries. KHCC is acknowledged as a leader in cancer treatment in the Middle East and its mission and vision is to maintain its leading position in cancer therapy . Hence, it embarked on establishing the first ISO accredited cancer biobank (KHCCBIO) for the purpose of collecting, processing, storing and distribution of cancer biospecimens that can help in the clarifying the disease cause and its effective treatment.

Currently, there are no or very few biobanks in the Middle East. However, KHCC changed this situation by establishing an ISO-compliant biospecimen repository (KHCCBIO) which incorporated all current international guidelines and best-in class practices under an approved quality management system to procure, process and store biospecimens for the benefit of researchers in Jordan, its neighboring countries, European researchers and researchers throughout the rest of the world. KHCCBIO bank follows the highest ethical standards in collecting, processing, storing and distributing these high-quality, clinically annotated biospecimens by consenting patients prior to collecting these samples and protect patients' rights and confidentiality.

KHCCBIO project was a 30 month project which started on 15/11/2011. KHCCBIO grant agreement was signed on the 29th of February 2012 and the EU pre-financing was received on March 19, 2012. Nevertheless; work started by holding the Kickoff meeting in November 19, 2011, as originally scheduled.

In brief, KHCCBIO bank approach is based on knowledge transfer between organizations and individuals from Europe and Jordan, in existing technological innovation and internationally recognized quality standards. Training courses on consent for tissue banking, procurement, processing and preservation was conducted by Trinity College Dublin , a college specialized in the management of thoracic malignancies. Training in ISO quality management, equipment and systems infrastructure qualification, tissue storage and distribution standards was executed by Biostόr Ireland, a fully accredited and EU-licensed tissue establishment. Future aims of the KHCCBIO support action are to develop KHCC’s research infrastructure, increasing its scope and visibility and improve its competitiveness throughout the European and worldwide biomedical science arena. Establish a platform for future knowledge transfer and collaborative research. Develop and enhance partnerships between European and Middle Eastern organizations for future staff exchanges, seminars and conferences focused on cancer. Disseminating KHCCBIO progress and activities particularly with EU early lung cancer (EUELC) collaborators to ensure the greatest impact and build competency in order to facilitate the participation of KHCC in future EU-calls.
Project Context and Objectives:
The main overall objective for the entire project period was to develop a biobank at KHCC for the purpose of collecting, processing, storing and distributing high-quality, clinically annotated biospecimens from consented patients that is compliant with international standards.

Description of the work performed and the main results achieved during the project were as follows.

WP 1: Coordinating Support Activities & Managing Project. KHCC successfully managed the KHCCBIO project throughout the project and took over the entire management part.

KHCC supervised and managed the achievement of the contractual deliverables and the milestones according to defined deadlines and submitted all scientific/technical reports on time. KHCC guaranteed prompt responses and disseminated information provided by the European Commission to the partners and arranged for telecons and webinars. Monitored work plan and partners activities and followed up on deliverables and sent reminders about work plan and progress to partners. Managed communication among partners and prepared a presentation on the project status and compiled a cost statement for the Technical Review at a project review meeting that was held in Brussels. KHCC was also responsible for the overall financial management of the KHCCBIO project whereas all partners are solely responsible for their own budgets and KHCC distributed the EC contribution to all partners according to the payment scheme laid down in the CA.

WP 2: Implementing Quality Measures. An ISO Quality Management System was established for KHCCBIO to operate according to it. Validation and Training Plans were written by KHCCBIO staff; reviewed & revised by KHCCBIO chief quality officer and deputy director and approved by KHCCBIO director.

WP 3: Supporting Policy Development. During the project period patient consenting policies, and forms for Consenting were written and approved for Ethics Application for Biobanking. Policies on Data Privacy & Confidentiality and Material Transfer Agreements, MTA’s policies were written to define rules for access to the bio-resource, ownership of biological samples and to ensure the highest standards of ethics and governance. KHCCBIO-Biobanking Bylaws were also written and approved. KHCCBIO staff was trained on the consenting process.

WP 4: Harmonising Sampling. SOP’s for the Procurement, Processing, Preservation and Disposal of blood and human tissue samples, based on existing Biobanking SOP’s in use within the Thoracic Oncology Biobank at TCD, in conjunction with international standards were written by KHCCBIO staff; reviewed & revised by KHCCBIO chief quality officer and deputy director and approved by KHCCBIO director then implemented by KHCCBIO staff.

WP 5: Assisting Verification. Installation Qualification (IQ) and Operation Qualification (OQ) protocols and draft Report formats for the identified equipment and systems from their existing Biostór QMS were adapted by KHCCBIO written by KHCCBIO staff; reviewed & revised by KHCCBIO chief quality officer and deputy director and approved by KHCCBIO director then implemented by KHCCBIO newly trained staff.

WP 6: Banking & Collaborating. SOPs for Thoracic tissue & associated blood sample storage and distribution were adapted by KHCCBIO written by KHCCBIO staff; reviewed & revised by KHCCBIO chief quality officer and deputy director and approved by KHCCBIO director then implemented by KHCCBIO newly trained staff.

WP7: Disseminating & Applying KHCCBIO bank & Capacity building. During the project period KHCC used several tools to disseminate the outcomes of the KHCCBIO project to guarantee its sustainability and arranged for KHCCBIO staff to train at EU-partners’ well established centers in this field for capacity building. KHCCBIO established a specific, unique and dynamic Website. It also wrote and distributed factsheets and prepared a Database of contacts, organisations, local/regional/national/pan-European authorities. KHCC also organized a final international conference titled “Conference on Cancer Biobanking: Establishing the First Cancer Biobank in Jordan,” which was sponsored by the FP7-EU program and considered as an initiative of KHCC with ESBB and WHO support. The conference was held under the patronage of HRH Princess Ghida Talal at the Le Meridian Hotel, Amman, Jordan on the 10th and 11th of April, 2014 to announce the inauguration of KHCCBIO. Several scientific papers related to the establishment of KHCCBIO were written and published in peer reviewed journals.

In brief, KHCCBIO bank approach is based on knowledge transfer between organizations and individuals from Europe and Jordan, in existing technological innovation and internationally recognized quality standards. Training courses on consent for tissue banking, procurement, processing and preservation will be performed by Trinity College Dublin (TCD), a college specialized in the management of thoracic malignancies. Training in ISO quality management, data privacy standards, equipment and systems infrastructure qualification, tissue storage and distribution standards will be executed by Biostόr Ireland, a fully accredited and EU-licensed tissue establishment.

8. BCNET INTERNATIONAL WORKING GROUP MEETING 25–26 SEPTEMBER 2013. International Agency for Research on Cancer, Lyon, France

The Low-and Middle-Income Countries Biobank and Cohort Network (BCNet) International Working Group meeting was held at the International Agency for Research on Cancer (IARC), Lyon, on 25 and 26 September 2013. The BCNet initiative arose from the realization that despite improvements in developed countries, population cohorts and biobanking facilities are either underdeveloped or non-existent in low- and middle-income countries (LMICs); thus, public health professionals and decision-makers lack the scientific evidence about cancer causes and control measures that is needed to set up targeted and cost-effective action plans within their health-science systems. In addition, many LMICs have yet to develop standard guidelines and protocols regulating the sharing and use of biological samples for research purposes.
The organizations represented in the Working Group included the Biobanking and Biomolecular Resources Research Infrastructure (BBMRI), the International Society for Biological and Environmental Repositories (ISBER), the European, Middle Eastern and African Society for Biopreservation and Biobanking (ESBB), the African Organisation for Research & Training in Cancer (AORTIC), the Latin American and Caribbean Biobank Network (REBLAC), the National Cancer Research Institute (NCRI), United Kingdom, the Population Project in Genomics and Society (P³G) and the Spanish National Cancer Research Centre (CNIO).
The meeting was attended by 23 participants representing 14 LMICs from all of the WHO regions (South-East Asia, Western Pacific, Africa, Europe, Middle East and Pan-America) and nine international organizations, as well as IARC staff members. During the two days, the participants discussed:
 The feasibility of and need for an LMIC network, what facilities are available, and whether there are initiatives that may overlap with BCNet.
 Specific aims and objectives, and short-term and long-term goals.
 IARC’s role in the network and advice on future steps.
 Possible sources of funding for individual and collective projects.

Potential Impact:
KHCCBIO is expected to have a major impact and contribute to medical advances by providing
high-quality human biospecimens and data to the cancer research communities world-wide.
KHCCBIO will contribute to RTD capacity building and management in Jordan and neighbouring countries, increase scientific excellence and efficacy of Jordanian research in biomedical sciences, and better integrate Jordanian science and medicine with its European neighbours resulting in enhanced participation of Jordanian organisation in future EU-Framework Programmes.

KHCCBIO will be the nucleus for the establishment of a national biorepository in the Hashemite
kingdom of Jordan which will be the first in the region. The KHCCBIO bank will increase the visibility of the King Hussein Cancer Centre within Jordan, the neighbouring countries and throughout the world, facilitating communication between centres of similar scientific interest. The KHCCBIO bank will disseminate scientific information as well as the results of research, and will result in networking opportunities for KHCC researchers with other research centres in EU Member States and Associated Countries.

The establishment of the KHCCBIO bank will encourage the next generation of researchers from
Jordan and the region to start collaborations with researchers from Europe at a much earlier stage in their career, which will enable them to place science within the context of development in the Middle East using the stored biospecimens in the KHCCBIO bank. In this respect, the KHCCBIO bank will provide better job opportunities and better access to research infrastructures for young scientists and avoid the current “brain drain” phenomena which is a central part of Jordanian career development. Furthermore, promoting the importance and benefits of such a biobank to the scientific and non-scientific community will advance research and establish personalised medicine as well as aid the development of new pharmacogenomic drugs. While the primary impact of the KHCCBIO bank is to ensure the delivery of improvements in the prevention, diagnosis and treatment of Thoracic cancer, the establishment of such a resource also represents a major investment in Jordan’s knowledge economy because it will facilitate cutting-edge academia/industry research and development. A vital effect of the biobank is to improve patient care; it is also important that patients as well as the public in general understand the purpose of the biobank and the role that patients and the public alike can play in making it a success.

KHCCBIO will collect material following the highest ethical and quality standards accompanied with extensive clinical information so that researchers global wide will be able to access this information via the online database that will be established in order to make use of these resources in their research. We further anticipate that KHCCBIO will make special tumour cell lines prepared from our ethnic background that covers the entire Middle East region and make them commercially available which will give pharmaceutical companies and researchers in Europe and the world opportunities to develop clinically sound hypotheses that will help identify new diagnostic and therapeutic targets.

KHCCBIO became partners with the Low-and Middle-Income Countries (LMIC) Biobank and Cohort Network (BCNet). BCNet provides a platform for collaboration between its partners, IARC and the international community, to support biobanking and cohort-building activities, and to develop sustainable biobanking research infrastructures to facilitate the collection of high
-quality biological samples for research, using best practice principles and guidelines.
