Clinical and economic outcomes of the two strategies (proposed care model and care as usual) will be compared in model based decision analyses (decision trees). The economic analyses will evaluate cost-effectiveness (costs per clinical outcome and per QALY) based on pooled data from the three trial centres (Kuopio, Amsterdam and Barcelona), as well as per centre.
