Resultado final

Report of the survey results (se abrirá en una nueva ventana)

Report summarising the results of the survey conducted in five countries (Italy, Spain, Denmark, Romania, and the UK) to identify the needs and key quality performance indicators of patients and carers, and healthcare professionals, in relation to managing multimorbidity.

Detailed methods documentation, including epidemiological and economics inputs and evidence used to inform the model.

This document will facilitate the software architecture desgins and this contain the early project requirements as co-morbidities are identified.

Report on the qualitative data (se abrirá en una nueva ventana)

Report presenting the findings of the qualitative work (focus groups and interviews) with mulitmorbid patients and their caregivers.

This will include a review of user feedback and conformity with data protection guidelines.

Report on the needs and QPIs (se abrirá en una nueva ventana)

eport of a standard set of outcomes and quality performance indicators and the tools/measures for these variables to inform the randomised controlled trial .

Report of a microsimulation model to perform effectiveness and cost effectiveness analyses.

The digital platform is the key deliverable for UNIMAN and will be used in the RCT.

This is the public project website for general communication purposes, both internal and external. A responsive design will be used to ensure a versatile use also on mobile devices. The public website will present AFFIRMO activities and results to the general public fostering Stakeholder & public engagement that will take place throughout the life of the whole project to engage other researchers in the project activities and present.

The interactive prototypes will support requirements workshops and will allow us to present our understanding of requirements in an interactive way, without the need to deliver software to do so.

This is a document outlining the requirements of the digital platform. Requirements will be provided at a high-level via D5.1, with the software requirements specification providing a more detailed description
