Report of the survey results

Report summarising the results of the survey conducted in five countries (Italy, Spain, Denmark, Romania, and the UK) to identify the needs and key quality performance indicators of patients and carers, and healthcare professionals, in relation to managing multimorbidity.

Detailed methods documentation, including epidemiological and economics inputs and evidence used to inform the model.

High-level requirements document

This document will facilitate the software architecture desgins and this contain the early project requirements as co-morbidities are identified.

Report on the qualitative data

Report presenting the findings of the qualitative work (focus groups and interviews) with mulitmorbid patients and their caregivers.

Digital platform quality assessment

This will include a review of user feedback and conformity with data protection guidelines.

Report on the needs and QPIs

eport of a standard set of outcomes and quality performance indicators and the tools/measures for these variables to inform the randomised controlled trial .

Microsimulation model

Report of a microsimulation model to perform effectiveness and cost effectiveness analyses.

Digital platform

The digital platform is the key deliverable for UNIMAN and will be used in the RCT.

Project public website

This is the public project website for general communication purposes, both internal and external. A responsive design will be used to ensure a versatile use also on mobile devices. The public website will present AFFIRMO activities and results to the general public fostering Stakeholder & public engagement that will take place throughout the life of the whole project to engage other researchers in the project activities and present.
