HIGH RESOLUTION LASER DOPPLER PERFUSION IMAGING IN DERMATOLOGY

Obiettivo

In the evaluation and treatment of a number of diseases and conditions that affect the skin, there is a need for cost-effective, objective and reliable medical technologies. An example of such a technology is High Resolution Laser Doppler Perfusion Imaging (HR-LDPI), which, originally intended for visualisation of disturbances in micro-vascular blood perfusion in peripheral vascular disease, now makes possible the visualisation of tissue blood perfusion in terms of two-dimensional flow maps. The LDPI utilises a low power laser beam that scans the tissue surface, this gives an image of new and valuable information about the heterogeneity in tissue perfusion which is many situations has a significant diagnostic and prognostic value. Its non-invasive nature, in combination with ease of use and rapid response, makes this technology well suited for laboratory as well as clinical investigations of skin blood perfusion and dissemination to new applications, for which it was not originally intended. The most important of these new applications are found within the discipline of dermatology and may have a substantial impact on a variety of diverse fields including patch testing, screening for malignancies, multiple parameter assessment in association with cutaneous microdialysis and development of skin care products.

The user needs within the four sub-projects listed above are not met by existing methods and procedures. Standardisation and harmonisation of the assessment and evaluation of various skin conditions and diseases are urgently required in order to meet these needs and to provide cost-effective and reliable alternatives to the procedures applied today.

The project aimed at improving the diagnosis of common skin diseases that may impair the quality of life and even lead to lethal consequences. These skin diseases which frequently influence people in the most active part of their life are to a large extent caused by environmental factors. Early diagnosis and efficient treatment may therefore have a substantial social and economic impact. Furthermore, early identification of potentially harmful substances in our environment is anticipated to play a major role in the prevention of these kind of diseases.

The reliability and effectiveness of HR-LDPI was demonstrated in the assessment of human skin within standardisation of irritant and allergy patch testing; discrimination of malignant and benign skin neoplasmas; visualisation of skin perfusion during evaluation by micro-dialysis of compounds which come into contact with the skin; and evaluation of skin perfusion in development of skin care products.
