General practitioners involvement in cancer prevention

Obiettivo

In only a limited number of countries the role of general practitioners in cancer prevention is thoroughly studied. The aim of this concerted action was to analyze differences in health services which might influence the overall effect of cancer prevention strategies with particular emphasis on the role of primary Health Care and general practitioners.
A network of researchers in general practice and cancer prevention compared from 1989 to 1992 general practitoners' involvement in cervical cancer screening between European countries and developed a consensus on rules for the organization of general practitioners involvement in cancer prevention.

For cervical screening the study has defined a number of more detailed principles for general practitioner involvement (Brix). In the area of breast cancer screening a more limited review was prepared. Most research was performed for these cancers in the countries participating in the study. Other areas for secondary cancer prevention, such as colorectal cancer screening and prostate cancer screening, remain controversial. Full scientific proof of their usefulness has to be provided before introducing large scale programmes with primary care involvement. Areas for primary prevention, such as smoking cessation and dietary advice, have to be further explored in the future. They may well meet higher priority among southern European countries than secondary prevention of cervical and breast cancer. Firstly this is due to incidence of disease. Secondly the lesser cost make these actions more feasible.

The results provide a framework from which practical applications can be evolved in each country according to the local health service setting. Verification of the forwarded hypotheses which were based on a interactive exchange process between researchers in 15 countries, is a major task for the future in each of them.
The following more specific objectives were formulated to implement the action:

1. Collect and compare data for different European countries concerning the involvement of general practitioners in cervical cancer detection as an example of cancer prevention through primary Health Care.
2. Formulate a consensus on high quality involvement of general practitioners in cervical cancer detection at European level.
3. Develop a more general model for promoting cancer prevention through primary health care.
