Improvement of hip fracture prediction in osteoporotic subjects by low-dose volumetric QCT assessment and finite element analysis of the proximal femur

Ziel

The objective is to improve hip fracture prediction in osteoporotic patients, as compared to the conventional dual X-ray absorptiornetry (DXA). Hip fractures in elderly reduce quality of life and increase morbidity and mortality. Local bone mineral density, cortical thickness and volumes, and geometrical parameters will be determined volumetrically by spiral quantitative computed tomography (QCT) using an optimised low-dose acquisition protocol. The parameters will be correlated with bone strength and fracture load in mechanical tests. Hip fracture discrimination will be investigated in 120 elderly females. Finite element models based on the QCT scans will be generated to optimise individual fracture prediction. Standardization and quality assurance will be ensured by a dedicated software and an anthropomorphic hip-phantom. Results will allow improve targeting of preventive treatments in selected patients with potentially high fracture risk.
